I'm guessing the empty page is due to the first chapter's starting on an odd page, so that LaTeX needs to insert a page to get to an odd page.
I'd suggest redefining your page style for the TOC. You've presumably already done that once---defined \lhead and the rest. Just define these right before the TOC, however you want them for the TOC, and then redefine them for the main matter.
