It uses a lot of ERT because we don't actually have a layout file for this. I 
put it on my to do list.

Attached is an example LyX-file. You haev to export the document as pdf(pdflatex) to get it correctly to work. Have a look into the preamble to set the pdftitle and pdfauthor.
