Well, your permissions look fine. Is there a possibility that some other application is using the temp directory when you exit? I'm not familiar with OS X. Is there, say, an indexing service that might be eyeballing the directory (and not letting LyX remove it)? Perhaps a virus or spyware scanner has it?
There's a utility that lets me view processes under Windows (something like top works under Linux, I think) and also lets me see which processes are accessing a particular file handle. If a similar utility exists for OS X, perhaps you could repeat your "hello" experiment and check who (if anyone) besides LyX is locking the temp directory.
