Good question. I have MikTeX 2.6, with the most up-to-date version of the lm package, installed on a PC and a laptop. On the laptop, I can use lm with no trouble, including math insets. On the PC yesterday, I could use lm for plain text, but it would not display math insets because it could not find/make the lmmi8 font. (I think it was 8, but I won't swear to that -- might have been 6.) So I updated formats and tried something else (I forget what), and today I get no output at all using lm fonts on the PC. I uninstalled/reinstalled the lm package, updated MikTeX's binaries, updated the font maps ... no joy. I have a tech-support (TeX-support?) question in to Christian Schenck.
Best guess is that there's a font-mapping problem on the PC. It would not surprise me if you were also experiencing a font-mapping problem, meaning that LaTeX wants to convert the math symbols (italic, possibly smaller font size than normal text) from the default cmr fonts to the appropriate lm fonts, but can't figure out the right one or can't find it. If I get an answer from Christian, I'll pass it along.
