I followed the instructions posted here:

for adding a caption to a longtable that runs over several pages. The
problem is that an entry shows up in the List of Tables for each page
of the table.
Solutions I tried:
1) the table is in an appendix, but removing the appendix environment
did not fix the problem.
2) instead of using "\caption[short title]{full title}\\%", I tried
omitting the [short title] part entirely with no results
3) using "\caption[]{full title}\\%" gets rid of the all of the
entries for this table in the List of Tables, which is not the
solution i'm looking for but may provide a work-around
4) I had the header "status: on" (ie, header repeating on each page).
Tried a) turning it to "status: off" and b) setting "first header,
contents: empty"; though neither removed the duplicate entries from
the List of Tables.
So I'm kind of stumped on this one and haven't been able to find a
solution from the mailing list archives. I've seen Any help would be
greatly appreciated.
