* Simplest: use page view instead of continuous view in the reader.
* Style the footnotes with one of the footnote related packages
bigfoot.sty, eskdfootnote.sty, fixfoot.sty,
footmisc.sty, mdwtools/footnote.sty,
ncctools/manyfoot.sty, ncctools/nccfoots.sty, pdfcolfoot.sty,
...
* Ask at comp.text.tex for help with hyperref jumping too low
(it always jumps to the baseline of the target, while the reader
displays the target at the upper margin of the window :-(
