Maybe a bug in 1.6.4 fixed in 1.6.5? Maybe miktex doing something that
texlive doesn't?
It is possible, but if larkc isn't found, then you should get an error.
I'd try loading LyX from a terminal and checking the LaTeX output when
you compile.

The problem is that the formatting is just plain 'book' class.
Do you mean in the LaTeX output, or in LyX itself? If the latter,
then this is what you should see. The layout does nothing but
import the layout for book. So it IS book, except for the fact
that you get, in the output:
\documentclass{yourclass}
or whatever, instead of:
\documentclass{book}
If you want to customize the appearance in LyX, then you need to
do more.
