Spent most of the day trying to figure this out, here's what's happening: If the Introduction (in frontmatter) is setup as a numbered Chapter, then LyX displays: ------- Chapter 1 Introduction Chapter 2 The First Chapter Title Chapter 3 The Second Chapter Title -------- On the other hand, if the Introduction is setup as an unnumbered chapter (Chapter*), LyX displays: ------- Introduction Chapter 1 The First Chapter Title Chapter 2 The Second Chapter Title -------- The first example Looks wrong in LyX, but prints correctly; the second example looks correct in LyX, but prints incorrectly, breaking all the chapter cross-references. -- Thanks!
