All I can say is that this is also the case in LyX 2.0.5 and that
BufferParams::moduleCanBeAdded() returns false for these modules. If
there's a rationale for this, I don't know.
I have no trouble adding "Theorems (Numbered by Section)" or
"...Chapter)" in 2.0.5 so long as one of the prerequisites is
available. What you can't add is "Numbered By Type within..." unless
you have " Theorems* (Numbered by Type)" selected.
I do see the problem in trunk.
