Replying a little more explicitly to your first question, "is that pinouts.txt file correct?", yes it is. I used it many times both to dump commercial option roms in a TL866 programmer, and while helping design the various FigTronix adapter boards which let you make your own option rom from a soldered-on 28C256 SOIC, or a socketed 28F256 PLCC32.
