Willard's reply is a classic example - two copies go out, because reply
all goes to both [email protected] and [email protected] (and
I see both).  Some people have mail infrastructure that detects the
duplicates (probably from the message ID) and discards them.  I'm not
sure why this happens - recipient and reply-to are both set to
[email protected] (there is a header
"X-Original-To: [email protected]", but mail readers shouldn't
be concerning themselves with that).
