There is a pure ram way to do it. I don't have a link handy but I remember the way it was described it poked a little routine into ram that ends up getting called every time some rom routine gets called, it detours through this added routine along the way to modify the result.
It sounded terrible except for the fact that there is no better way without editing the rom.
