IT's very confusing to me when I try to use ICS with it, maybe it makes more 
sense in older versions?

You say  you like the combination of EyesFree shell and Spiel. I just can't get 
my head around EyesFree shell. All the letters seem to dance around the screen 
and I couldn't use it at all. I wonder if I am not understanding how it really 
works. I have tried it with Talkback but not Spiel at this stage. I mostly use 
MA because I seem to get further with that though it only seems to work on 
certain apps. Is that how it should be?

Have you tried any of the free accessibility solutions instead of Ma?
Like, EyesFree shell in combination with either Talkback or Spiel?
I personally like EyesFree Shell with Spiel, The interface has some nice 
touches.
You have to do some customization of the program to get it to your liking, but 
it doesn't suffer from the problems inherent in the Ma applications.
It uses the apps on your phone already for contacts, calendar, messages, etc.

And, very important, at least on my phone, EyesFree shell, Spiel and SVOX 
together, take up much less space on my phone than Ma does.
