If your Motorola phone was released in 2011 or later and will not be
getting upgraded to ICS let alone Jelly bean, complete the form anyway.
If all they give is a credit toward the purchase of another Motorola
phone, then you can choose to accept that or not. Since you may not be
able to use the upgrade, you got a 2011 device late in 2011 or early
2012 before they reached end of life and can't upgrade, you may get a
rebate instead and that is something that could benefit you. In my
case, My phone the Cliq2 isn't getting upgraded and since Motorola
doesn't have a phone that t-mobile sells anymore, they will have to give
me the rebate, which I shall use to purchase the latest phone running
Jelly bean on t-mobile either on contract or off and be done with that
phone, unless I can root it and in that case, I will have a nice backup
or can use the rebate elsewhere. I can see them making that decision as
gingerbread on my device and running mobile Accessibility is laggie and
prone to prolong freezes and performance degradation.

yeh, motorola is good for that. i have a motorola charm that was
supposed to receive one more upgrade and was supposed to get it
shortly after we purchased two of them to fnd shortly there after that
the upgrade was not coming. we are going to upgrade to the samsung s3
and get away from motorola product.

phone, I'll be going with something else when that time comes when
I'm granted permission from Sprint to upgrade.
