Hello Ian, I am greatly relieved to report that the problem has been solved. Though your diagnosis was incorrect, it got me to thinking, and I resolved the problem in a pretty non-spectacular way.
First of all, I went to the Sounds entry in settings and determined that my sound volume was at 100%. Then, I turned off the phone and waited for a minute before powering it up again. No change. Finally, while holding the volume up key, after unplugging the earphones, I just started randomly touching the screen to make VO talk. All of a sudden, I started hearing the speaker. This leads me to conclude that the external speaker volume is independent from the earphone volume. Perhaps this will help someone else, too.
