Hello David, I am using bookmarks organised by folders on Lion and also used them on Snow Leopard. Below is how I am set-up on Lion ....
In the bookmarks tab in Safari preferences I have all the options unchecked except "Include bookmarks bar bookmarks menu" which is checked. When on a web page for which I want to record a bookmark I go to the Safari bookmarks menu and choose "add bookmark". If I press the tab key a few times I encounter a pop up box which lists all the bookmark folders I have created. I select the desired folder and then press tab to go to the edit box for the desired bookmark name and enter that and finally tab twice more to locate the add button and press that. When I later want to go to a bookmarked page I go to the Safari Bookmarks menu and choose "show all bookmarks". I then use the VO cursor to locate two tables one before a search area and one after the search area. The table before the search area lists the bookmark collections. I can use the "New Collection" button at the end of the above menu to add a new named collection to the list of collections. I interact with the collections table to locate the desired collection and then stop interacting. The second table then shows the bookmark folders in the chosen collection. I can use the "New Folder' button at the end of the above menu to create a new folder in the chosen collection. I can interact with the second table and then use the arrow keys in the same way as for the Finder Column view to locate folders and expand desired folders. I can press the delete key to get rid of bookmarks and folders which are no longer required. I can use Command+x and command+v (i.e. normal cut and paste) to move bookmarks between folders. I can select a folder name I do not like and overtype it to change that name. Likewise I can select a bookmark name I do not like and overtype that. I hope the above helps....
