Whilst your iPhone is speaking something press on the up volume button on the phone, see if that raises the level. Other methods, if you have a keyboard connected to your phone - bluetooth or dock doesn't matter - hold down ctrl-option and command keys, use left and right arrow keys to cycle through Voiceover Options, volume will be in amongst them. When you cycle to volume, keep hold of ctrl-option and command keys and use the up/down arrows to adjust Voiceover volume.
