The only way I know to do this is using the OpenOffice plug-in and it's really 
clunky and messy.  I tried it once and gave up because of the huge holes in the 
plug-in and the severely restricting way that it's been implemented.  It's 
possible to create a DAISY book using synthetic audio based on text, but you 
cannot easily insert publisher notes, chapter markers etc. wherever you want 
them.
