Hello Andy, If you don't repair permissions regularly, your Mac can start misbehaving such as losing the sounds in Mail, going busy too often, having short cuts stop working, and all kinds of other problems I've heard people complain about. I've been using Macs for 17 years, long before VoiceOver, and I rarely have problems but I also repair permissions regularly and reset the PRAM every month or so. What you don't need to do on a Mac is defrag the HD. Housekeeping is the first job a Mac does when powered on, or if left to sleep, sometime in the early hours of the morning.
