The way I learn about these modified menu options is by luck, when I am reading about a particular topic, or by navigating to each menu option, then press Option key by itself to see if VO announces anything. If the menu changes when I press the Option key by itself, I can activate that new menu item by still holding down Option and pressing Enter. An example is when I navigate to the Apple menu, and come down to Shutdown, I then press Option key for a moment, and VO will say "Shutdown" again, and then I press Enter while still holding down the Option key. This will let me shutdown without any additional confirmation prompts.
