I have checked back and it is indeed now command plus control plus shift plus T to add an item to the Dock in Mavericks.

Hi Ian, To remove things from the dock, go to the tiem you want to remove and hit vo m. Arrow down til you see options. vo space on the options menu and remove from dock is one of those options.
