It sounds like his phone was not removed from his iTunes account. The one he gave to his wife. I believe the process that should have been taken was to remove it from his account and restore the phone to original status. Also remove it from find my IPhone if he uses that. Then his wife could have set it up as a new iPhone and put her information in there and it would have just put her things on it and not his. Just a thought.
