You certainly can with OS X, the trick is those routers. Are these garden variety home units? or?
If you could set up port forwarding on those units it will work fine. If not it won't be easy...

Since most cable modem setups use a dynamically assigned IP to the router, you need to find out the current IP that has been assigned to your router by the cable ISP. Your friend needs to enter that IP into the server address box in the "connect to server" dialog under the "go" menu in finder. Your friend can then log in either as a guest, which gives access only to your "public" folder, or log in as a registered user to an account that you've set up on your machine (which could be your account).
