There are several questions in this:

You can re-install Address from the OS-X install disk, you will need an 
application like
Pacifist

http://www.versiontracker.com/dyn/moreinfo/macosx/12743

or unpkg

http://www.versiontracker.com/dyn/moreinfo/macosx/26406

to get the thing extracted from the install CD.

Then run one of the combo updaters from Apple to make sure that the 
latest updates for Address book are applied.

Before doing that, try running Repair Permissions on your drive to see 
if that fixes it.

The preferences for Address Book are buried inside of the Application 
folder itself and are not normally for the causal user to go after. But 
if you hold down the control key and then click on the application, you 
will get a contextual menu popping up that allows you to view the 
contents of the  "application" -- it really is a special type of folder 
with all of the goodies inside, cruise on in and hunt around and you 
will find the preferences.

The Address Book data lives inside of your Application Support folder 
inside of YOUR user folder. Try moving this Address Book data folder to 
the desktop and then see if AB will run. You may have some address 
listing that is somehow messed up and causing problems.

Note: if you navigate to the main library folder (not the library 
folder in your user library, but the main system library folder) you 
will find a folder called Receipts. Open this up and search through it 
for a receipt for MacOSXUpdate10.3.5.pkg delete this and then you can 
run the 10.3.5 installer again.

But it might be a better idea to run a combo updater to get you to a 
later version of Panther. In spite of a small spate of troubles for 
some, many folks are not having any problems whatsoever with 10.3.9
