There aren't any known defects in Mach-II that I know about that would cause this behavior. That doesn't mean under the right conditions that something could crop up. I'd suspect based on the little information going on that it would be a race condition situation going on otherwise it would fail *each* time or consistently fail under certain condition. That's just a guess.

What version of Mach-II are you using currently?
