I had this problem with a rented movie a while ago. Even after it timed out after having watched it, iTunes would download it again every time I deleted it and then restarted or checked for downloads. I had to finally write to the iTunes Store support to get them to manually remove it from their end. No idea how that one happened, but definitely annoying.
