In our previous episode (Thursday, 28-Mar-2013), Jeff Weinberger said:

That has never been associated with your AppleID nor used to make a purchase on 
your account. This is important. And the second factor authentication only 
kicks in when you ADD the account. So, if you have a device that has been 
associated with your AppleID before, it's not going to bother you with the 
two-factor thing.

Basically, it's designed to be as least annoying as it can possibly be.
