Personally, I'd be in favor if ditching the mysql port if it duplicates mysql3. But personally I use mysql5, so I may be missing something... ;)

"mysql3" port:
 * lists more official-looking mirrors
 * has some more options passed to configure
 * uses startupitem for +server

"mysql" port:
 * uses DarwinPortsStartup for +server
 * claims support for sunos.
