I use the combination of

getmail
maildrop <> DSpam
dovecot

maildrop is available via Macports
http://www.courier-mta.org/maildrop/

As I also couldn't find a way of having Sieve work in Dovecot via MacPorts. Also Dovecot is now at 1.1.7 (MacPorts is at 1.1.3).
