I don't know what Xquartz is but looking at X11 preferences there is an option to emulate a 3 button mouse. Wonderful! I've activated this and now, at least with gv which is the only place I've tested it, left-click, middle-click and right-click are obtained by using click, alt-click and cmd-click.
