Do you have cairo or cairo-devel? The latest cairo (which is newer than the 
latest cairo-devel) provides a sufficiently new library.

    % port installed 'cairo*'
    The following ports are currently installed:
      cairo @1.12.14_0+x11 (active)
    % otool -L /opt/local/lib/libcairo.2.dylib | head -n 2
    /opt/local/lib/libcairo.2.dylib:
