As for creating the boxed edition, you'll have both design and printing costs, and it really depends on what you're after. In 1000 unit quantities, the printing will probably run you $3-5 per box depending on how thick you want your box stock, what kind of coating you need, whether you need a divided compartment for a manual, etc... A good commercial printer can walk you through all the options. I would recommend using a local printer if possible so you can attend the press check. I used Lahlouh (http://www.lahlouh.com/) for the boxes and Far Western Graphics (http://www.farwesterngraphics.com/) for the manual.
