Been there, done that, got one 10 years ago. (Actually I have 
two, one for my consulting business and one for my software business.)

A Trade Name Certification from the Arizona Secretary of State 
is exactly like a DBAs from other states.

The *very* first thing I did was to FAX Apple my Trade Name 
Certification. The problem is that a DBA or Trade Name 
Certification is NOT on Apple's list of acceptable documents, 
running me head-first into a bureaucratic stone wall.
