Thanks, Doug. That sounds like what I'll be doing.

It's official, I just heard back from Apple: Apple does not 
consider a Trade Name or Doing Business as (DBA) Certificate as 
acceptable documentation of a company. If you can't produce a 
business license, you can't have an iPhone developers account as 
a sole proprietor.

So after chasing this for six weeks, I'm back to submitting my 
app as an individual.

*sigh*
