That's been my experience too, until today.

I talked with a developer services representative and their 
supervisor for about 10 minutes. The supervisor (supposedly) 
also talked to their supervisor, and the story was the same: 
Apple legal has determined the acceptable forms of documentation 
and they can't make any exceptions. If you can't supply one of 
the forms that list, you can't get a company iPhone developers accounts.

I've wasted far too much time banging my head against this 
particular obstacle. It's time to relocate my head to the next 
immovable object.
