Jumping in here with my 2 cents. I have operated as a sole proprietor since 1981 and usually without a business license as I do not live in a town or city. As I recall, I sent in my federal tax ID and sales tax number and registered as PolyMicro Systems in the developer program.
I have never seen a real need to incorporate since as a sole employee the protection offered in next to nothing.
