Ken,

Try and enable CONFIG_MAC_EMUMOUSEBTN=y.
Then use sysctl to set sensible values for the stuff in 
/proc/sys/dev/mac_hid/*

So for instance set the button2 keycode to shift and the button3 keycode 
to alt. Then you can use Shift-Click for the second button.
