Okay, last night I installed 2.6.22-rc5. The combined_mode has gone. You 
can remove it from your kernel command line. Make sure you do not 
compile in the old ATA drivers - they can't handle the hard drive and 
compete with SATA for grabbing the device. Compile in the SATA drivers 
only - they can handle the PATA CD-ROM now, too. You can find a working 
config here (32bit):

By the way, the iSight driver works again with this kernel!
