Solved the mystery. Option up and down are actually Quick Nav
commands. Voiceover did not announce when pressing the left and right
arrows. Fixed that so now I know when Quick Nav is on. Scrolling does
not function when Quick Nav is off.

the Home Screen, Option plus Left or Right will move to previous or next page.
In a list, Option plus Up or Down will scroll the list.
