Yes, VO doesn't always focus on the item for the shortcut menu. You can press option-enter to download a linked file. Also, if you go into accessibility preferences with 5 times command-option-f5, you can enable mouse keys. This is a function that doesn't depend on vO, and uses the numpad to move the mouse pointer. You can toggle it on and off by pressing the option key five times quickly. Once you have mouse keys on, you can press control-numpad-5 on the link to do a right-mouse-click. This second method is a good option for me, since I always have mouse keys enabled.
