Hi,

In the Users & Groups pane of System Preferences, you may have accidentally 
deleted the text for your Display name.  Your "Short Name" cannot be removed, 
so the account remains OK.  You can fix this by going into the Users & Groups 
pane again, clicking the lock at the bottom of the pane and entering your 
password to unlock this pane, then you should be able to navigate around and 
find the field for the text of your Name.  What you've done by changing the 
format to actually require typing of both your name and password will do the 
trick but you may wish to fix the Display name as well.
