Ok I'm going to give you a condensed answer to your questions, and  
that's why I'm top posting here.  So here goes.
1. About the size, I'd recommend looking into some of the cases  
available for the nano.  They may help add girth.  I can't recommend  
one yet but I'm looking at the iSee clear case or the incas clip,  
although it has mostly bad reviews.  I like that it has a clip on it  
though.  the iSee clear case has a headphone wrap on it.
2. In itunes when you connect the ipod you can uncheck the options to  
include videos, and podcasts, movies and whatever else you don't  
want.  in the main ipod screen there are tabs for each.
3. Be sure to check "Read Menus under the voiceover settings to have  
VO read the menus, or you'll just get clicks.
