I wasn't sure who the culprit was. When I started seeing this problem, Safari had just been upgraded and LastPass had been upgraded fairly recently too. I will have to try the maximize trick next time I need to use it. I'm surprised this doesn't affect people not using VO as it seems to grab keyboard focus. But I have other focus issues with Safari too so why I haven't tried contacting anyone at Last Pass yet.
