Hi, Steve,

Well, I cheated.  I just asked my web developer to create a fillable PDF form 
for me.  My plan had been to try and create it in Word, which in many ways 
would have been my preference, but I changed my mind due to the concern you 
mention about Mac users who don't have Word installed, because I wasn't sure 
how Pages would handle it.  I would love to see a good option for this on the 
Mac.
