I suspected something and tested it.  Could it possibly be that when you went 
to press CMD+o, the usual Add to library dialog did open up but in another 
window and because VO didn't announce the fact, you missed it?  Sounds silly, I 
suppose:  but, that is pretty much what happened to me just now.  I pressed 
CMD+O and was not focused where I expected to be.  All menus were dimmed out 
thereafter.  Guessing what had happened, I kept pressing the close button until 
everything (that is all open windows) had closed.  Got out of iTunes and went 
back in and tried it again.  Works now.
