To add to what Tim said (which was completely correct, of course), you can also 
merge cells from the context menu which you activate with VO-shift-m. However, 
whichever method you use, note that
VoiceOver will not announce that the cells are merged if you hold down the VO 
keys to navigate; and
VoiceOver will not read the contents of the merged cells if you use only arrow 
keys to navigate.
This usually means navigating with only arrow keys, but if you come across 
merged cells you'll need to use the VO keys and/or VO-F3 to read the contents.
