Hi there 
It's not free, but there is an app called PDF Pen that works with VoiceOver. 
There are some restrictions. They haven't got checkboxes to work yet, so you 
may have to get a reader to do those. The other thing is that if this form is 
not set up with accessibility features, the option to find the form fields, so 
I've heard, doesn't always put the form field in the right place. If the form 
is set up with accessibility features, then it works fine. I have done tax 
forms with PDF Pen. I have the Pro version.
