In relation to the reported table of contents issue I have not seen this, certainly not in any iBook. However I can confirm difficulty with the alternative method of chapter navigation by keyboard. short cut. In your case I wonder if the book you are trying to read with its associated table of contents is a properly formatted iBooks or epub book. On other platforms I have found some books created in ePub format by conversion programs sometimes have problems with table of contents structures.
