Quick correction: the Mac does include this option, as of 10.10 I believe. In 
System Preferences, go to Accessibility, then choose Descriptions from the 
table, and Find and check the box for "play audio descriptions when available". 
I haven't tested this, as none of the iTunes movies with descriptions interest 
me, but the option does exist.

On iOS, some people have found that the app will take your video description 
preference into account and will use descriptions without you needing to choose 
them if that setting is enabled. I can't confirm if this is true or not, but 
that's one thing I've heard.
