I am not experiencing your first problem. I am running the latest iOS and am 
writing this message using a ,refreshabraille 18 braille display.

As for your last problem, go to settings, general, accessibility, voiceover, 
braille. Writing and reading braille are seperate selections. I have my braille 
input set to 8-dot computer braille and my reading output set to ,,ueb 
contracted braille.

Whilst writing this message, I am noticing that, each time I press space or a 
punctuation symbol, my braille display shows the beginning of the line. Whilst 
writing each word, the display shows the word I am currently writing.
